We can only accept BC/BS for private insurance. If you have different private insurance, you may pay cash and we will provide a receipt for you to submit to your insurance for possible reimbursement. For individuals covered by Medicare Part B, you must bring your Medicare and/or Medicare Advantage card with you for billing purposes.
Masks MUST be worn and short sleeve or loose-fitting shirts are suggested (shoulder accessible).
You will be asked to remain on-site for up to 15 minutes for observation after receiving your vaccination.
